C-FC Health Office Guidelines
Required before the first day of school:
- Proof of Immunization or appropriate waiver on file per WI state law
- Proof of age signature page
Recommended for School Entry:
- Physical exam completed by medical Provider
- Dental exam completed by dentist
- Vision exam completed by MD or optometrist
Medications at School:
- School Medication Procedure Form and signed Physician orders
- Prescription in original, labeled package
- Medications are stored and administered from the health office unless released for field trips
School Attendance:
- Children with a temperature at or above 100.4, persistent/productive cough, earache, rash, upset stomach, diarrhea, pink eye, ring worm, strep throat, chickenpox, scabies, head lice, etc. should stay home until treated (if contagious) AND symptom free for 24 hours. Children with a fever should not return to school until they have been fever-free for 24 hours without the use of fever-reducing medications.
- If your child has a contagious condition, notify teacher and nurse so precautions can be taken to protect the other children
Head Lice:
- Check child’s hair and scalp routinely – home is the best place to stop the spread of lice. By the time lice is identified, the lice have been present up to a month or more.
- Educate your child on the importance of not sharing hats, combs, etc.
- Children will be sent home until treatment is provided and child is free of lice and nits. Following treatment, parents may bring their child back to school and nurse/ staff will check to ensure treatment was effective and no lice/nits remain.
Health Office Functions:
- General illness/injury triage and assessment
- Promote health education and connect families to resources
- Conduct Health Screenings and maintain records as required by state law
- Medication administration
