You will need the following information:
- Household information - address and phone numbers
- Parent information - work and cell phone numbers, email addresses
- Student information - demographic and health/medication information
- Emergency Contact - addresses and phone numbers
- Student vaccination record
- Name and address of last school attended (if applicable) and most recent report card or high school transcript (if available)
- A valid email address
Students enrolling in school for the first time will need to also provide the office with a Birth Certificate.
