Student Accident Insurance

Students are not covered by an accident insurance policy through the school district. As a service to parents, the school district does make available to parents an accident insurance plan through Student Assurances Services, Inc. of Stillwater, MN.

The Student Assurance Services plan provides for benefits for medical services incurred because of an accident. A premium envelope with an explanation of the plan is handed out to all elementary students on the first day of school and is available at the Junior High Orientation. Junior-senior high students may stop in the office for an application or download the form from this site.

Parents who wish to apply for this coverage should read the benefit information listed below explaining the program. Print your name, address, and other information clearly on the application. Include a check for the proper amount of money (please do not send cash) made out to Student Assurance Services. Mail the application to Student Assurance Services.

Coverage begins on the day SAS receives your application and premium. Insurance coverage may be purchased at any time throughout the school year.

It is especially important that you check into this insurance if your student is in athletics. In grades 7-12, a waiver is included on the co-curricular form which needs to be signed indicating you have sufficient insurance or are purchasing insurance through Student Assurance Services.
